Immigration Physicals

All applicants for permanent residence in the United States of America have to file a "Medical Assessment" that can be performed only by assigned doctors by the Department of Homeland Security and the Immigration and Naturalization Service.

Dr. Raul Ayala is certified "Civil Surgeon for Immigration Medical Examination," which allows him to perform all the necessary examinations, as well as to issue a certification that you need for your case.

For your appointment, you need to bring your immunization record, if not here, we can administrate you the vaccines needed, provide you the form and will help you fill it out. Click here to download Form I-693.

The exams to be taken are:

  1. A physical exam.

  2. Proof of Tuberculosis (TB) for all those over 2 years of age. You must come back for the results 3 days after. The result is considered "positive" when there is a reaction to 5 millimeters or more.
    Anyone who gives "positive" to the test must be subjected to x-ray of the chest.

  3. Evidence of HIV (the virus that causes AIDS)

  4. A blood test for syphilis (RPR)

  5. Immunization routine like chickenpox, MMR (measles, mumps, measles) and tetanus-diphtheria.

  6. For children of 14 years of age or younger the exams consists on the following:

  7. oPhysical examination

  8. oEvidence of tuberculosis

  9. oRevision and updating of pediatric vaccines

Medical Certification for FAA (Federal Aviation Administration)

All applicants to pilots, air traffic controllers and aviation students need to obtain a medical certification that can ensure the good health to perform these tasks.

Dr. Ayala has the license to the U.S. Department of Transportation Federal Aviation Administration as "Certified Aviation Medical Examiner, Class II and III" to perform the appropriate exams.

Medical Certification for the Department of Transportation (DOT)

Anyone with work occupation related to drive a commercial motor vehicle must have the proper medical certification in order to obtain the license.

Dr. Raul Ayala is one of the doctors licensed by the Department of Transportation for such testing and certification. The same is valid for up to 24 months, or it could be less time when a patient requires monitoring his/her health periodically.
